AVR and LoopOut card are probably not compatible.
LoopOut cards usually have problems, better is a splitter with scaler and EDID settings + grabber.
Quote from Yamaha HTR-4068The unit is compatible with the video signals of the following resolutions:
• VGA
• 480i/60 Hz
• 576i/50 Hz
• 480p/60 Hz
• 576p/50 Hz
• 720p/60 Hz, 50 Hz
• 1080i/60 Hz, 50 Hz
• 1080p/60 Hz, 50 Hz, 30 Hz, 25 Hz, 24 Hz
• 4K/30 Hz, 25 Hz, 24 Hz
• 4K/60Hz (YCbCr4:2:0), 50Hz (YCbCr4:2:0)
• When CPPM copy-protected DVD-Audio is played back, video/audio signals may not be output, depending on the type of the DVD player.
• The unit is not compatible with HDCP-incompatible HDMI or DVI devices. For details, refer to the instruction manual for each device.

OK fixed it..
HTR-4068 has only one HDMI IN with HDCP support and STB was there plugged in.. STB does not need HDCP..
so switched HDMI IN cables from STB to SHIELD and it magically started to work correctly..
just one point.. CEC does not work..
I guess connection of PIN 13 from HDMI IN to PIN 13 of HDMI OUT in Video Grabber will solve it..

there is also a option to enable CEC in Hyperion.NG

Yes, PIN 13 is not connected through, as with most LoopOut cards.
